Job Description:
The hospital is currently searching for 4 Full-Time, Admissions-Discharge Coordinators for the Case Management Unit. Must have a valid California License and open to work any open shifts.
Reporting directly to the Director of Nursing, Medical-Surgical/Telemetry Services or designee, the licensed nurse working in the Admission-Discharge Nurse role is responsible for supporting the admission, discharge, and transfer process to facilitate patient throughput in the organization. Works with Nurse Directors, Nurse Managers, Nursing Supervisors, Clinical Supervisors, Charge Nurses, Bed Coordinators, and Intake Staff to facilitate patient progression and flow at the unit level to achieve system efficiency.

QUALIFICATIONS
General Job Requirements
- Registered Nurse (RN) Degree from an accredited school of nursing, college or university.
- Current California RN license.
- One to two years recent acute care or long-term care (subacute or LTAC) experience preferred.
- Experience in team nursing model work environment preferred.
- BCLS certification required
- Fire Card certification required.
- Solid assessment and care planning skills.
- Proficiency in the care planning process.
- Outstanding collaboration and communication skills, verbally and in writing.
- Proficient in computer operations using Microsoft and Excel software.
